Chapter 64 observes that people often fail when success is close. At the beginning, uncertainty prompts preparation, questions and spare capacity. Near the end, attention moves to celebration and the next task, fatigue rises, anomalies become “small issues”, and untested extras are added to make the result more impressive. Four common failures in the final stretch
First, “90% done” is mistaken for “only 10% of the risk remains”. Second, essential reviewers leave early. Third, new requirements enter just before the deadline. Fourth, delivery, migration, notification, training and cleanup are treated as minor tails outside the real work. Completion means a user can safely obtain the result and dependent states agree, not merely that the main file exists.
Turn equal care into four actions
Restate the end state: which person gets which result through which entry. Freeze non-essential additions and protect the verification window. “No failure” is not a magical guarantee
Care cannot eliminate disasters, another party's breach or genuinely unknowable events. The line works better as risk discipline than as blame for a person who suffered failure. Equal care reduces preventable errors; it does not abolish chance or make under-resourced people personally responsible for systemic defects.
